Job summary

Integrated Services for Behavioral Health (ISBH) in Pike County, OH, is seeking a Care Coordinator to join our community‑based team. You will work with families to identify care coordination needs, engage with children and youth, and link them to services in home, school, and community to promote healthy outcomes.

Qualifications

• Experience providing services to children and families in behavioral health or related fields.
• Knowledge/experience in wraparound services preferred (certification provided).
• Two years in a care coordination or supportive services role preferred.

Responsibilities

• Joins with family to identify care coordination needs and ensure outcomes.
• Works with families to define cultural factors influencing engagement.
• Coordinates family-based services in home, school, and community.
• Ensures services align with safety and treatment needs of child and family.
• Maintains documentation and participates in program planning and evaluation.
• Remains current with required trainings.
