Clinica Family Health in Lafayette, CO is seeking a Care Coordinator for our Integrated Health Home program. This full-time, on-site role supports clients facing complex mental health and substance use challenges through case management, counseling, and community services.

The ideal candidate has a Bachelor’s degree in Psychology, Social Work, or Counseling or 1 year of behavioral health experience, with skills in MI, DBT, and CBT.

Qualifications

• Bachelor's Degree in Psychology, Social Work or Counseling or one-year experience in behavioral health.
• Experience working with adults in an outpatient setting and interest in mental illness and substance use.
• Experience with Motivational Interviewing (MI), Dialectical Behavior Therapy (DBT), and other Cognitive Behavioral Therapies (CBT).
• Collaborative and client-centered orientation.
• Manage a caseload of clients with diverse needs.
• Familiarity with Medicare and Medicaid.

We are seeking an enthusiastic and empathetic Care Coordinator to join our team. In this role, you will provide essential support and guidance to a diverse clientele facing complex mental health, substance use, and environmental challenges.

• Do you have your Bachelor in Psychology, Social Work or Counseling and looking to get more experience working in mental health settings?
• Are you passionate about helping clients focus on health and wellness treatment goals and services work with a vulnerable client population?

What We Need:

• Bachelor's Degree in Psychology, Social Work, Counseling or a related field OR one-year experience in the behavioral health field for non-related Bachelor's degrees
• Experience working with adults in an outpatient setting, interest in working with adults with mental illness and substance use issues
• Experience with clinical modalities: Motivational Interviewing (MI) and Dialectical Behavior Therapy (DBT), other Cognitive Behavior Therapies (CBT)
• Collaborative and client centered orientation
• Manage a caseload of clients and their diverse needs
• Collaborative team member who is ready to jump in, support our clients and team
• Community outreach experience and interest, occasional ability to transport clients to connect to care
• Familiarity with insurance programs like Medicare and Medicaid
