Job summary

Coordinated Family Care in New Jersey is seeking a Care Manager to coordinate and provide assessment, planning, and care management services to youth and families, in a respectful, dignified manner that builds on family strengths.

The role requires a Bachelor's in a Human Service field, 1+ year nonprofit youth/family experience, strong communication and teamwork, and a valid driver’s license with access to a vehicle; shifts 8am–7pm, Mon–Fri, in community or office settings.

Qualifications

• Bachelor's Degree in a Human Service field.
• At least one year post undergraduate/graduate experience providing family and youth services within a nonprofit social services setting.
• Excellent communication, facilitation, conflict-resolution, assessment and system negotiation skills.
• Ability to engage youth and work as a team.
• Must have a valid driver's license and access to a vehicle.
• Must be available to work in the community and/or the office for an 8-hour shift during core hours of 8am-7pm, 5 days a week Mon-Fri to meet the needs of the families served.

Salary Range: $52,500 - $57,500 annually
