Job summary

Avantara Palos Heights, located in Palos Heights, IL, is seeking a Falls/Psychotropic Registered Nurse (RN) to join our compassionate team. You will screen residents on psychotropic medications for safety, monitor reactions, and coordinate psychiatric services to uphold high standards of care.

Applicants should hold an unencumbered Illinois RN license and CPR certification, with about 1 year RN experience in a skilled nursing setting preferred.

Qualifications

• Unencumbered Illinois RN license and CPR certification.
• 1 year RN experience in a Skilled Nursing Facility preferred.
• Demonstrated commitment to resident safety and high standard of care.

Responsibilities

• Screen and assess residents on psychotropic medications for appropriateness.
• Monitor adverse reactions and evaluate potential for drug reductions.
• Complete MDS documentation and evaluations, obtain informed consents, and develop care plans for residents on psychotropic meds.
• Coordinate psychiatric services to support resident well-being.
