Job summary

Avantara Palos Heights, located in Palos Heights, IL, is seeking a Falls/Psychotropic Registered Nurse (RN) to join our compassionate team. You will screen residents on psychotropic medications for safety, monitor reactions, and coordinate psychiatric services to uphold high standards of care.

Applicants should hold an unencumbered Illinois RN license and CPR certification, with about 1 year RN experience in a skilled nursing setting preferred.

Responsibilities

• Coordinate psychiatric services to support resident well-being.

As a Falls/Psychotropic Nurse you are the backbone of the Clinical Department. Your work will ensure our residents receive the high standard of care they have grown to expect at Avantara Palos Heights by screening and assessing all residents on psychotropic medications for clinical appropriateness, monitoring for adverse reactions and evaluating potential for drug reductions, completing MDS documentation and evaluations, obtaining informed consents, completing care plans for residents on psychotropic medications and coordinating psychiatric services. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Illinois Registered Nurse (RN) license and C.P.R certification. One (1) year of RN experience in a Skilled Nursing Facility setting is preferable.
