Job summary

Segue Health is a faith-based medical practice delivering in-home and community-based care in the Meridian area. We seek a compassionate Care Coordinator to manage post-discharge transitional care, coordinating with hospitals and referral facilities to ensure smooth handoffs.

The role blends clinical knowledge with strong organization and communication, including documenting EMR data, scheduling home visits, and making follow-up calls within 24 hours. Competitive benefits are provided.

Qualifications

• Must have experience in the medical field.
• Must be proficient with Microsoft Excel and capable of learning EMR/EHR software.
• Should have reliable transportation to local hospital.
• Must have a heart to serve others.
• Knowledge of demographics and post-discharge processes; daily logging of referrals and discharges.
• Experience in social work, nursing, or home health is preferred.
• Attention to detail is essential.
• Vaccination proof or compliance with hospital vaccination requirements.

Responsibilities

• Collaborate with referring facilities to identify patients eligible for transitional care management.
• Visit referrals in facilities to explain the TCM program and assess post-discharge needs.
• Log referrals and discharges daily.
• Create EMR charts and upload face sheets, H&P, and DC summaries.
• Make post-discharge calls to patients within 24 hours.
• Schedule in-home visits prior to discharge when possible.
