Job summary

Segue Health is a faith-based medical practice serving DeSoto County and surrounding areas. We seek a compassionate Care Coordinator to manage post-discharge transitional care, coordinate with facilities, and arrange in-home visits.

Ideal candidates have medical field experience, Excel and EMR/EHR familiarity, strong organizational and communication skills, and willingness to travel locally. Vaccination requirements apply; competitive benefits provided.

Qualifications

• Experience in the medical field.
• Proficient with Microsoft Excel and able to learn EMR/EHR software.
• Transportation to and from local hospital.
• Strong attention to detail and organizational skills.
• Experience in social work, nursing, or home health preferred.
• Vaccination proof or compliant with hospital vaccination requirements.
• Daily tasks include coordinating post-discharge transitional care (TCM), visiting referrals, logging referrals and discharges, creating EMR charts, post-discharge calls within 24 hours, and scheduling in-home visits.

Responsibilities

• Coordinate post-discharge transitional care with facilities.
• Visit referrals to explain the TCM program and determine post-discharge needs.
• Log referrals and discharges daily.
• Create EMR charts and upload face sheet, H&P, and DC Summary.
• Make post-discharge calls within 24 hours of discharge.
• Schedule in-home visits prior to discharge.
