Segue Health is seeking a compassionate Care Coordinator to join our faith-based medical practice serving patients in Franklin, TN. The role involves coordinating post-discharge transitional care management with strong organizational and communication skills.

Responsibilities include working with facilities to identify eligible patients, visiting referrals, logging referrals and discharges in EMR, creating charts, and arranging in-home visits when possible.

Qualifikationen

• Must have experience in the medical field.
• Must have knowledge of Microsoft Excel and the ability to learn EMR/EHR software.
• Should have transportation to and from local hospital.
• Must have a heart to serve others.
• Our most successful candidates have experience in social work, nursing, and/or home health.
• Attention to detail and organizational skills are MOST important.

Aufgaben

• Working with referring facility to generate list of patients eligible for TCM services
• Visiting referrals while in the facility to explain the TCM program and determine if there are any anticipated needs post discharge
• Daily logging of referrals and discharges
• Creating charts in EMR and uploading face sheet, H&P, and DC Summary
• Post discharge calls to patients within 24 hours
• Scheduling of patients preferably prior to discharge for in home visits

Segue Health, a faith based medical practice, is seeking a compassionateCare Coordinator to join our team serving patients in the Franklin, TNarea coordinating post discharge transitional care management. This position requires a mix of clinical knowledge, excellent organizational skills, and excellent communication skills.

Qualifications

• Daily tasks include:

Job Type: Full-time

Salary: $50,000.00 - $60,000.00 per year

• 8 hour shift
