Serenity Hospice PA is seeking an experienced Hospice RN Case Manager for a dedicated facility-based position in West Chester, Pennsylvania.

This is a unique opportunity to case manage hospice patients while being assigned to just one facility. • Participate in interdisciplinary team meetings and communicate changes to patient plans of care

  • Foster a strong, professional partnership between Serenity Hospice and the facility
