Job description

Overview

Family Support Program (FSP), formerly known as the Individual Care Grant program, provides access to intensive mental health services and supports to youth with a severe emotional disturbance. The goal of the FSP is to support eligible youth and their families by strengthening family stability, improving clinical outcomes, and promoting community-based services.

The Specialized Family Support Program (SFSP) is a 90‑day program of crisis stabilization, community mental health, and assessment services. It is designed to identify the behavioral health needs of youth at risk of custody relinquishment (i.e. psychiatric hospital lockout) and to link those youth to the most appropriate clinical services.

Basic Job Function

Under the supervision of a Licensed Practitioner of the Healing Arts (LPHA), the Coordinator may provide some or all of the following: clinical assessment, treatment planning, crisis intervention, coordination with the Illinois Department of Healthcare and Family Services (HFS) and residential entities, collaboration with hospital staff, case management, community support, therapy/counseling, and administer direct clinical interventions to children, adolescents, and their families experiencing acute and chronic emotional/behavioral disorders. The FSP/SFSP Coordinator will provide these services in a variety of settings—primarily within the home or community.

Reports To: Clinical Supervisor and Behavioral Health Manager

Principal Duties/Responsibilities

• Working knowledge of FSP criteria and requirements. Ability to communicate and coordinate with potential FSP candidates in order to enroll them into the program. Once approved, maintain their enrollment and link the recipient to community based and/or residential based services. Maintain weekly verbal communication with client and family to ensure stabilization goals are met and maintained. Complete and submit continued enrollments, prior authorizations, and applications as needed in accordance with HFS protocol.
• Working knowledge of SFSP criteria and requirements. Facilitate enrolling youth into the program, gather all required documentation, and coordinate with the hospital staff until the youth is discharged into the parents’ care. Provide support and direct services to the family for 90 days to maintain stabilization in the home. Complete required HFS reports.
• Perform Integrated Assessments and Treatment Plans (IATP) for children and adolescents referred for FSP/SFSP. This includes the ability to develop and successfully provide a diagnostic assessment consistent with all Axis of the current Diagnostic and Statistical Manual and International Classification of Diseases, or their successors. Ensure that all services are in compliance with standards of informed consent, maintenance of client rights, and standards of confidentiality.
• Maintain direct service implementation included in the client’s treatment plan. Engage client in clinical therapeutic rehabilitative services, case management, community support, crisis intervention, and mental health stabilization.
• Provide consultation with other professional service providers in accordance with the current administrative code.
• Prepare and oversee case presentation for clinical review at specified points of service, including initial assessments, treatment planning review, treatment staffing reviews and case closure.
• Ensure that all clients are linked with appropriate treatment, educational, social, recreational, and medical resources during and prior to completion of service.
• Maintain current knowledge regarding services to children, adolescents, adults, and families. Participate in agency in‑service trainings, as well as self and supervisor directed advanced skill‑trainings. Maintain and adhere to current rules and regulations pertaining to services delivered by the Illinois Department of Human Services, Department of Children & Family Services, Illinois Healthcare & Family Services, including the Abused and Neglected Child Reporting Act (ANCRA), and Office of the Inspector General.
• Participate at an advanced level with colleagues, agency consultants, intra and inter‑agency service coordination, and community‑planning and advocacy on behalf of clients served.
• Document service provisions consistent with the treatment plan and ensure that all identified services are provided in accordance with Agency/Program, and current administrative codes for the delivery of community mental health treatment services. Maintain client confidentiality and adhere to guidelines defined by the Health Insurance Portability and Accountability Act (HIPAA).
• Perform other duties commensurate with the responsibilities outlined herein.
• Maintain current and accurate documentation of client services (billable/non‑billable), client scheduling, consultation, and other events related to the maintenance of the client’s electronic health record.

Specifications

This position requires a minimum of a master’s degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, family therapy, or a related field as defined by the governing mental health authority.

Must have successfully completed a practicum and/or internship which includes a minimum of 1,000 hours, or one year of clinical experience under a Licensed Qualified Mental Health Professional, including QMHP physician’s, licensed clinical psychologists, or registered nurses licensed under the Illinois Nursing Act of 1987 with at least one year of clinical experience in a mental health setting.

The position requires the use of a personal auto, a valid driver’s license, and a Motor Vehicle report acceptable to the agency’s broker and verification of insurance coverage.
