Job summary

Sans Souci Rehabilitation & Nursing Center is seeking an Experienced Physical Therapist to provide direct patient care in a long-term care setting. The PT will assess patients, develop and administer treatment plans, document progress, and educate families on therapy methods.

The role requires a valid state license, graduation from an accredited PT program, and the ability to supervise PT Assistants. Experience in geriatric care and coordination with a multidisciplinary team is preferred.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid state license/registration.
• Oversee Physical Therapy Assistants.
• Geriatric/long-term care experience preferred.
• Strong documentation practices.
• Conduct evaluations, progress notes, recertifications and discharge summaries.

Responsibilities

• Assess referred patients requiring rehab and provide direct patient care.
• Develop and administer individualized treatment plans with goals based on patient tolerance and physician guidance.
• Document patient condition and progress; educate patients and families on therapy methods.
• Lead and supervise Physical Therapy Assistants.
• Collaborate with the interdisciplinary team to ensure patient-centered care.
• Maintain HIPAA compliance and patient confidentiality.
• Report to work on time and maintain professionalism.
