Job summary

Sans Souci Rehabilitation & Nursing Center is seeking an experienced Physical Therapist to evaluate and treat patients in a long-term care setting. The PT will assess functional status, design therapy plans, and monitor progress in collaboration with the physician.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed and registered to practice physical therapy in the state of practice.
• Experience supervising Physical Therapy Assistants.

Responsibilities

• Assess referred patients and provide direct patient care.
• Develop and administer individualized treatment plans with goals.
• Document patient condition and progress, including weekly notes and discharge summaries.
• Educate patients and families on therapy methods and home programs.
