Job summary

Sans Souci Rehabilitation & Nursing Center is seeking an experienced Physical Therapist to provide direct patient care, assess needs, and develop rehabilitation plans. The PT will document progress, educate patients and families, and supervise PTAs while collaborating with the multidisciplinary team.

The role emphasizes adherence to confidentiality, HIPAA, and residents' rights, with competitive compensation and benefits. Prior long-term care experience is preferred.

Qualifications

• Graduation from an accredited Physical Therapy program or university.
• Current valid license and state registration to practice.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care/geriatric settings preferred.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Establish and administer treatment programs with goals aligned to physician guidance.
• Document patient condition and progress; educate patients and families.
• Supervise and collaborate with PTAs and other disciplines; ensure documentation compliance.
• Maintain confidentiality and adhere to HIPAA and residents' rights.
