Hospice of San Joaquin is seeking an experienced RN Case Manager in Stockton, CA to support palliative and hospice care within a dynamic interdisciplinary team. Responsibilities include nursing assessments, care planning, patient education, and coordination with physicians and caregivers in home or facility settings.

The role requires California RN licensure, strong communication skills, and the ability to manage high-stress situations.

Qualifications

• Graduate of an accredited school of nursing in California; a Registered Nurse.
• Three years of recent clinical experience.
• Computer skills and ability to learn clinical software.

Responsibilities

• Provides Palliative education, emotional support, and community resource information to patients and families.
• Completes initial nursing assessment and develops measurable treatment plans.
• Maintains up-to-date patient records in EMR and adheres to Medicare and state standards.
