As an Experienced Hospice LPN at h/care, you will be an integral part of our patient care team, focused on delivering quality nursing practices alongside emotional support tailored to each patient's needs. This role requires a combination of medical expertise, compassion, and excellent interpersonal skills to ensure a comforting environment for both patients and their families.
Duties and Responsibilities
- Provide direct nursing care in accordance with the patient’s care plan consistent with the scope of the state's Nurse Practice Act.
- Monitor patient’s condition and notify appropriate personnel of any changes.
- Administer prescribed medications, change dressings, clean wounds, and monitor vital signs.
- Collaborate with the nursing team to optimize patient care and comfort using a holistic approach.
- Maintain accurate patient medical records and document nursing interventions.
- Educate patients and their families on disease processes, medical-surgical procedures, and other aspects of therapeutic regimens.
- Advocate for the patient's comfort and satisfaction, adjust care strategies accordingly.
- Engage in decision making related to the appropriateness of healthcare interventions.
- Facilitate timely communication between patients, family members, and the multidisciplinary hospice care team to ensure continuity of care.
- Participate in hospice quality assurance and performance improvement initiatives.
- Undertake ongoing formal and informal education to maintain and enhance nursing skills and knowledge.
Requirements
- Must hold an active LPN license in the state of employment.
- Minimum of two years’ experience in a hospice or related health care facility.
- Dependable with a commitment to maintain scheduled hours required.
- Strong desire to make a significant difference in the lives of patients and their families.
- Exceptional bedside manners, reflecting empathy and commitment to patient care.
- A proactive 'get anything done' mentality geared towards patient care and team collaboration.
