Job Opening: Experienced Hospice - LPN (Aurora, Hudson, and Stow)

h/care is excited to announce an opening for a dedicated and compassionate Experienced Hospice - Licensed Practical Nurse (LPN). This is an outstanding opportunity for an individual passionate about providing vital end-of-life care in a hospice setting. The position available is full-time and requires a committed presence onsite, as this is not a remote opportunity. As an Experienced Hospice LPN at h/care, you will be an integral part of our patient care team, focused on delivering quality nursing practices alongside emotional support tailored to each patient's needs. This role requires a combination of medical expertise, compassion, and excellent interpersonal skills to ensure a comforting environment for both patients and their families.

Duties and Responsibilities
  • Provide direct nursing care in accordance with the patient’s care plan consistent with the scope of the state's Nurse Practice Act.
  • Monitor patient’s condition and notify appropriate personnel of any changes.
  • Administer prescribed medications, change dressings, clean wounds, and monitor vital signs.
  • Collaborate with the nursing team to optimize patient care and comfort using a holistic approach.
  • Maintain accurate patient medical records and document nursing interventions.
  • Educate patients and their families on disease processes, medical-surgical procedures, and other aspects of therapeutic regimens.
  • Advocate for the patient's comfort and satisfaction, adjust care strategies accordingly.
  • Engage in decision making related to the appropriateness of healthcare interventions.
  • Facilitate timely communication between patients, family members, and the multidisciplinary hospice care team to ensure continuity of care.
  • Participate in hospice quality assurance and performance improvement initiatives.
  • Undertake ongoing formal and informal education to maintain and enhance nursing skills and knowledge.
Requirements
  • Must hold an active LPN license in the state of employment.
  • Minimum of two years’ experience in a hospice or related health care facility.
  • Dependable with a commitment to maintain scheduled hours required.
  • Strong desire to make a significant difference in the lives of patients and their families.
  • Exceptional bedside manners, reflecting empathy and commitment to patient care.
