Reports To: Clinical Supervisor, Home Health

FLSA Status: Non-Exempt

Role Summary: As an LPN on our home health team, you’ll provide skilled nursing care in patients’ homes, collaborate with interdisciplinary partners and educate families. Your focus on safety, empathy and clear communication ensures every visit supports patient independence and comfort.

Key Responsibilities

  • Patient Assessments & Care

  • Perform vital signs, wound care and medication administration per care plan

  • Educate patients and families on self-care strategies and medication use

  • Coordination & Documentation

  • Work closely with RNs, physicians and therapists to coordinate seamless care

  • Complete accurate, timely EMR documentation after each visit

  • Safety & Compliance

  • Follow infection-control protocols, HIPAA rules and state/federal regulations

  • Handle medical supplies and equipment safely

  • Team Collaboration & Growth

  • Participate in case conferences, team huddles and peer mentoring

  • Pursue continuing education to stay current with best practices

A Typical Day

  • Review patient assignments and gather supplies

  • Travel to home visits, deliver nursing care and build rapport

  • Document findings and update care plans in real time

  • Check in with your supervisor or RN for feedback and hand-offs

  • Use any downtime for training, chart maintenance or case reviews

Qualifications

  • Valid Ohio LPN license and graduate of an accredited LPN program

  • Minimum 1 year of home health or equivalent clinical experience

  • Proficient in wound care, medication administration and patient teaching

  • Familiar with EMR systems (e.g., Kinnser, McKesson) and mobile charting

  • Strong communication, time-management and problem-solving skills

  • Comfortable adapting to varied home environments and schedules

Work Environment

  • Fast-paced, patient-centered field role with office check-ins as needed

  • Flexible schedule including occasional evenings or weekends
