Job summary

Horizon House in Seattle is seeking a Director of Nursing (DON) to lead clinical operations across Assisted Living and Memory Care, setting standards for care and regulatory compliance. You will supervise nursing staff, oversee RN delegation, care planning, and change-of-condition evaluations, while partnering with the leadership team to drive quality and safety across a 250-employee site.

Qualifications

• Active RN license in good standing with the State of Washington.
• 2+ years of recent experience working with a senior population.
• 2+ years of recent RN Delegator experience.
• 3+ years supervising 10+ direct reports in a healthcare environment.
• Current CPR certification and BLS training.
• Background working with dementia, aging-related needs, and complex family dynamics.

Responsibilities

• Serve as the primary clinical authority for the single-site community with Assisted Living and Memory Care.
• Lead the RN Delegation Program, including resident eligibility, task delegation, competency checks, and regulatory compliance.
• Lead, develop, and supervise Medication Technicians and nursing staff.
• Oversee care assessments, care planning, admissions, annual reviews, and change-of-condition evaluations.
• Manage incident reporting, investigations, documentation, and trend analysis.
• Ensure survey readiness through audits, corrective actions, and documentation integrity.
• Set clear expectations for clinical excellence, documentation, and safe practices.
• Partner with the Assisted Living Manager to align clinical and operational performance.
• Lead clinical quality assurance initiatives and performance improvement efforts.
• Strengthen medication management systems, infection prevention practices, and safety protocols.
• Track and resolve clinical grievances with timely follow-through.
• Partner with pharmacy, hospice, home health, and other providers to ensure coordinated care.
• Participate in committees and policy development to support organizational goals.
• Collaborate with Life Enrichment leadership to reinforce Memory Care practices.
• Develop and deliver training related to delegation, medication systems, safety, emergency procedures, and clinical risk management.
• Assess team learning needs and implement targeted training and coaching.
• Provide education to residents and families on clinical systems and care processes.
• Participate in the manager-on-call rotation to support clinical decision-making.
