Key Responsibilities

• Provide strategic leadership and operational oversight for Integrated Care Management programs across hospital and ambulatory settings
• Lead multidisciplinary teams including RN Care Managers, RN Care Coordinators, and Licensed Social Workers
• Build strong partnerships with clinical leaders, community organizations, and cross-functional stakeholders
