Job summary

Only Care Home Health in Pasadena, CA seeks a passionate Director of Nursing to lead our home health clinical team and drive high-quality patient care in patients' homes. This role requires strategic leadership, regulatory savvy, and a track record of building strong clinical programs.

The successful candidate will oversee nursing staff, ensure Medicare/Medicaid CoPs compliance, mentor RNs/LVNs and aides, and collaborate with physicians to optimize care plans and outcomes.

Qualifications

• Active RN license in California (required).
• Minimum 2–3 years of home health supervisory or management experience.
• In-depth knowledge of Medicare/Medicaid CoPs.
• Proven track record in quality assurance and clinical program development.
• Valid California driver's license and reliable transportation.

Responsibilities

• Oversee all clinical operations and nursing staff within the home health agency.
• Ensure compliance with federal, state, and local home health regulations and Medicare/Medicaid standards.
• Develop, implement, and evaluate clinical policies, procedures, and quality initiatives.
• Supervise, mentor, and evaluate RNs, LVNs, and home health aides.
• Collaborate with physicians and care teams to coordinate patient care plans.
• Monitor patient outcomes and lead QAPI programs.
• Manage staffing, scheduling, and resource allocation.
• Serve as liaison between clinical staff, administration, and regulatory bodies.
