Job summary

Johns Hopkins Care at Home is seeking an Executive Director to lead the design and implementation of the home care coordination and discharge planning model. The role interfaces with JHM/affiliates, external community organizations, and JHCH to optimize the care continuum and ensure efficient home-based services across Medicare, Medicaid, military, and commercial products.

Reporting to the Chief Operating Officer, you will oversee network administration for home care providers, drive utilization

Qualifications

• Master's degree in health-related field.
• 10 years progressive healthcare management experience.
• 5 years executive leadership.
• Health care and home care experience.
• Utilization review/management experience.

Responsibilities

• Leads design and implementation of home care coordination and discharge planning models to facilitate referrals and acceptance.
• Directs coordination of home-based services for plan products and ensures network sufficiency and appropriate utilization.
• Collaborates with hospitals, health plan leaders and affiliates to integrate care continuum and achieve strategic goals and budgeted volumes.
• Drives performance via KPIs and reports results to executive leadership.
