Job summary

Qualifications

• Masters in health-related field is required.
• 10 years progressive healthcare management experience.
• 5 years executive leadership experience required.
• Health care and home care experience required.
• Utilization review/management experience required.

Responsibilities

• Leads design and implementation of home care coordination and discharge planning to facilitate referrals to Johns Hopkins Care at Home Services and the Home Care Network.
• Interacts with JHM/affiliates, external community organizations, and JHCH to design care processes and improve care continuum throughput.
• Manages network home care providers to meet length of stay and utilization goals and reports KPIs to leadership.
• Maintains regulatory compliance and effectiveness of services to support JHCH and JHM needs.
