Nealy Pierce, LLC in Coastal Georgia is seeking a Director of Care Coordination (RN) to provide system-wide leadership over case management, utilization review, discharge planning, and palliative care coordination across two hospital campuses. You will guide a large team of about 65 staff and collaborate with hospital medical staff to optimize patient progression.

Qualifications

• RN required.
• Leadership experience in Case Management or Care Coordination within a large hospital or health system.
• Five to seven years in acute care hospital settings, with at least three years in leadership.
• Master’s degree required; if not in Nursing, must hold a BSN.
• CCM certification strongly preferred.

Responsibilities

• Oversee Case Management and Utilization Review system-wide.
• Lead discharge planning and length-of-stay performance.
• Coordinate Palliative Care and Social Work services.
• Direct a department of about 65 team members across two hospital campuses.
• Manage two site-based Case Management Managers and a Utilization Review Manager.
