Assist with the coordination of admissions, including reviewing and processing patient information, conducting resident assessments, and communicating with patients, families, and physicians to ensure a smooth transition into our facility.
Review and verify patient information, including medical history, insurance coverage, and other relevant data to ensure accurate and timely admissions.
Utilize PCC and Allscripts systems to manage patient information and admissions processes.
Collaborate with the nursing staff, and other healthcare professionals to ensure a smooth transition into our facility.
Communicate effectively with patients, families, and healthcare providers to address concerns and provide education on our services.
Participate in quality improvement initiatives to continuously improve the admissions process and patient outcomes.
Minimum of 3 years of experience in Nursing Home admissions, with a strong background in patient care and communication.
Proficient in PCC and Allscripts systems.
Excellent communication, customer service, and organizational skills.
Ability to work in a fast-paced environment and prioritize tasks effectively.
Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
Must have an outgoing personality and strong customer relation skills.
Ability to review and prioritize patient records (PRIs) preferred.
