Job summary

RYZE on the Avenue in Chicago, Illinois is seeking an experienced Nursing Supervisor to oversee resident care and staff, ensuring high-quality care for residents. The role supports evening or weekend shifts and requires strong leadership within an interdisciplinary team.

Responsibilities include assessing residents, documenting changes, coordinating with physicians and families, and guiding onboarding and ongoing education for clinical staff. Competitive benefits are offered.

Qualifications

• Graduation from an accredited School of Nursing with 5 years of experience as a long-term care nurse.
• Minimum of 2 years supervisory experience in acute, sub-acute, or long-term care.
• Licensed Registered Nurse in Illinois.
• Proficient in computer use and facility programs.

Responsibilities

• Oversee resident care during shift and report incidents to Administrator and Director of Nursing.
• Actively participate in committee/programs as directed by Director of Nursing.
• Oversee resident care to promote highest physical, mental, and psychosocial functioning for the unit.
• Perform nursing assessments and communicate significant changes to physician, family, or responsible party.
• Complete rounding to ensure resident care needs and environmental standards are met, including dining monitoring during meals.
• Collaborate with interdisciplinary care team to provide holistic care.
• Review clinical records for completeness and accuracy.
• Monitor, track, evaluate, and report infections.
• Act as a resource for nursing staff and review admissions for the unit with Director of Nursing and Admissions Director.
• Collaborate to identify orientation and continuing education for clinical team members.
