Qualifications

• Graduation from an accredited School of Nursing with 5 years of experience as a long-term care nurse.
• Minimum of 2 years supervisory experience in acute, sub-acute, or long-term care.
• Licensed Registered Nurse in Illinois.
• Proficient in computer use and facility programs.

Responsibilities

• Oversee resident care during shift and report incidents to Administrator and Director of Nursing.
• Actively participate in committee/programs as directed by Director of Nursing.
• Oversee resident care to promote highest physical, mental, and psychosocial functioning for the unit.
• Perform nursing assessments and communicate significant changes to physician, family, or responsible party.
• Complete rounding to ensure resident care needs and environmental standards are met, including dining monitoring during meals.
• Collaborate with interdisciplinary care team to provide holistic care.
• Review clinical records for completeness and accuracy.
• Monitor, track, evaluate, and report infections.
• Act as a resource for nursing staff and review admissions for the unit with Director of Nursing and Admissions Director.
• Collaborate to identify orientation and continuing education for clinical team members.

Job description

We are seeking an experienced Nursing Supervisor to join our team at RYZE on the Avenue in Chicago, Illinois. As an Evening or Weekend Nursing Supervisor, you will be responsible for overseeing resident care, managing staff, and ensuring the highest level of quality care is provided to our residents.

Shift:

3-11 weekdays and/or 7am-7p or 7p-7a Weekends

Essential Duties and Responsibilities:

• Assume responsibility and accountability for resident care during shift, ensuring complete and prompt reporting of incidents to Administrator and Director of Nursing
• Actively participate in committee/programs as directed by Director of Nursing
• Oversee resident care to promote the highest level of physical, mental, and psychosocial functioning possible for assigned unit
• Perform nursing assessments regarding the health/status of residents and ensure that significant changes in resident condition are communicated to the physician, family, or responsible party
• Complete rounding throughout the facility to ensure resident care needs and environmental standards are met, including monitoring of dining room during meal times
• Work collaboratively with other members of the interdisciplinary care team to provide holistic care
• Review clinical records for completeness and accuracy as necessary
• Monitor, track, evaluate, and report infections
• Act as a resource for nursing staff and review applications for admission to the unit with Director of Nursing and Admissions Director
• Collaborate with Director of Nursing to identify and provide orientation and continuing education for clinical team members

Qualifications:

• Graduation from an accredited School of Nursing plus 5 years of experience as a long-term care nurse, with a minimum of 2 years supervisory experience in acute, sub-acute, or long-term care
• Must be licensed as a Registered Nurse in the State of Illinois
• Proficient in the use of computer and programs utilized by the facility
• Ability to communicate in English both orally and in writing
