Job summary

Guild Services in West St. Paul, MN, seeks a Licensed Practical Nurse to support the Dakota County IRTS program. The LPN administers medications, conducts nursing duties, and collaborates with the RN and treatment team to meet residents’ health needs.

The role requires a Minnesota LPN license, at least 1 year of direct nursing care, and experience with psychotropic medications and EMAR/EHR systems. Evening shifts and weekend coverage are included.

Qualifications

• The candidate must have a diploma or certificate from an accredited practical nursing program.
• Current, unrestricted Licensed Practical Nurse license with Minnesota Board of Nursing.
• Minimum of 1 year of direct nursing experience as an LPN.

Responsibilities

• Administer and document medications following rights, policy, and state requirements.
• Transcribe and verify medication orders in EMAR; inform RN of changes.
• Monitor for medication effectiveness and adverse events; report to RN/prescriber.
• Provide health/medication education to clients individually and in groups.
• Support safe self-management of medications per treatment plan.
• Assist in training unlicensed staff on medication monitoring and EMAR.
• Maintain accurate documentation in EHR/EMAR; ensure regulatory compliance.
• Participate in treatment team meetings and share health observations.
