Job summary

Hospice and Palliative Care Buffalo is hiring Registered Nurses for our after-hours nursing team in Buffalo, NY. Full-time positions with evening and weekend schedules offering home and facility-based patient care.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license in the State of New York.
• Valid driver's license and access to a vehicle for community travel.
• One year of professional nursing experience preferred.
• Ability to assess patient needs and coordinate care across settings.

Responsibilities

• Provide compassionate hospice and palliative nursing care during evening, weekend, and after-hours hours.
• Respond to patient and family needs in home care and facility-based settings.
• Perform comprehensive nursing assessments addressing physical, psychosocial, environmental, safety, emotional, and spiritual needs.
• Assess and manage pain, symptoms, and changes in patient condition.
• Develop, implement, and update individualized plans of care as needed.
• Educate patients, families, and caregivers regarding symptom management and hospice services.
• Collaborate with physicians and the interdisciplinary team to coordinate care.
