Responsibilities

• Provide comprehensive services to clients in housing, entitlements, skill building, and peer support.
• Liaise with family, agencies, guardians, and natural support systems to enhance community inclusion and participation.
• Perform duties under Program Supervisor with emphasis on independent, person-centered planning.
• Ability to physically perform tasks and meet essential demands including lifting and flexibility.
