Qualifications

• Current LPN/LVN license in California.
• One year of experience in a long-term care setting.
• Basic Life Support (BLS) certification.
• Ability to lift, push, and pull up to 50 pounds.
• Strong communication and interpersonal skills.

Responsibilities

• Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities.
• Develop and maintain personalized care plans, working closely with the care team to ensure resident needs are met.
• Assist with mealtime, including serving, feeding, and ensuring hydration needs are met.
• Participate in care conferences and contribute to resident assessments, care plans, and service plans.
• Maintain accurate and timely records of resident care and services.
• Collaborate with other healthcare professionals, including physicians, CNAs, and staff members to ensure high-quality care.
• Engage with residents, families, and staff members to promote a positive and inclusive environment.
• Perform other duties as assigned by the shift leader or director of nursing.
