Job summary

SNF DIVISION in Ohio is seeking a compassionate LPN or RN for the day shift to deliver skilled nursing care, assess residents, administer medications, and update care plans.

You will document accurately, communicate with physicians, families, and the interdisciplinary team, supervise CNAs, and ensure infection control and safety standards in a fast-paced environment.

Qualifications

• Must hold an active LPN or RN license in Ohio.

Asa member ofour nursingteam, you willbe responsible fordelivering skilled nursingcare, assessing andmonitoringresidents' health status, administering medicationsand treatments asprescribed, and responding promptlyto changes incondition. You willensure accurate andtimely documentationof all nursingcare, communicate effectivelywith physicians, families, and interdisciplinaryteam members, andparticipate in developingand updating individualizedcare plans.

Additional responsibilitiesinclude supervising andsupporting Certified NursingAssistants (CNAs), ensuringcompliance with infectioncontrol practices andsafety protocols, completingadmissions, discharges,and transfers asneeded, and maintainingcompliance with stateand federal regulations, facility policies, andnursing standards. Youwill assistwith wound care, medication management,physician orders, andresidenteducation while fosteringa positive, respectful, and team-orientedwork environment. Thesuccessful candidate willdemonstrate excellent clinicaljudgment, strong organizationalandcommunication skills, theability to prioritizemultiple responsibilitiesin a fast-paced environment, anda genuine passionfor delivering compassionate, high-quality care.
