Job summary

Willow Springs Memory Care in California is seeking an Evening Shift LPN to join our person-centered memory care team. The role focuses on compassionate, skilled care for residents living with cognitive impairment and their families, with direct patient care and active collaboration with the care team.

You will develop personalized care plans, assist with daily living activities and mealtimes, and participate in care conferences to ensure high-quality services and resident well-being.

Qualifications

  • Current LPN/LVN license in the state of California.
  • One year of experience in a long-term care setting.
  • Basic Life Support (BLS) certification.
  • Ability to lift, push, and pull up to 50 pounds.
  • Strong communication and interpersonal skills.

Responsibilities

  • Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities
  • Develop and maintain personalized care plans, working closely with the care team to ensure resident needs are met
  • Assist with mealtime, including serving, feeding, and ensuring hydration needs are met
  • Participate in care conferences and contribute to resident assessments, care plans, and service plans
  • Maintain accurate and timely records of resident care and services
  • Collaborate with other healthcare professionals, including physicians, CNAs, and staff members to ensure high-quality care
  • Engage with residents, families, and staff members to promote a positive and inclusive environment
  • Perform other duties as assigned by the shift leader or director of nursing
