Job summary

Sunflower Communities is seeking an Evening Caregiver in Elmore, MN for Elmore Assisted Living. The role supports residents in Assisted Living and Memory Care with ADLs and evening routines, in a team-focused, family-owned environment.

The position offers 2:00 PM–10:30 PM shifts, every other weekend, with an evening differential and potential bonuses. Growth opportunities into CNA roles are available with company backing.

Qualifications

• Experience as a CNA, HHA, DSP, PCA, or Caregiver preferred.
• Willingness to pursue CNA certification with support provided.

Responsibilities

• Assist residents with ADLs including bathing, dressing, dining, and mobility.
• Help create a positive and structured evening routine.
• Provide support in Assisted Living and Memory Care settings.
• Monitor resident well-being and communicate updates to the care team.
• Document care and support a safe, comfortable environment.
