• Care Needs & Planning: implement and revise care plans, monitor health conditions, respond to emergencies or status changes.
• Staff Oversight & Scheduling: supervise care assignments, ensure coverage, evaluate staff performance, educate and lead staff.
• Family & Team Liaison: communicate changes in resident conditions to families and providers; collaborate with the interdisciplinary team.
• Documentation: review and audit incidents, transcribe orders, ensure record-keeping accuracy and compliance.
• Family, Resident, Staff Education: explain health status and care changes; guide and train care skills as needed.
