Qualifikationen

Aufgaben

• Conducts clinical rounds daily; assess health status of residents.
• Provide/coordinate direct resident care including care plan review, assessments, and medication review.
• Monitor care for at-risk residents, labs, and physician order changes.
• Address family issues and coordinate admissions, discharges, and transfers.
• Ensure staffing meets guidelines and supervise staff; orient agency staff.
• Maintain documentation accuracy and communication across the care team.
• Respond to emergencies and support nursing leadership.
