Job summary

Riverwood Health and Rehabilitation is hiring an evening shift RN/LPN to provide high-quality nursing care and collaborate with a multidisciplinary team to implement individualized care plans. The role focuses on patient-centered care, education for patients and families, and ensuring smooth coordination of services.

The ideal candidate is a Florida-licensed RN or LPN with at least 1 year of experience and CPR certification.

Qualifications

• Currently licensed as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Florida.
• Minimum of 1 year of experience in a healthcare setting.
• Current CPR certification.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and disease management.
• Conduct thorough patient assessments, develop and implement individualized care plans, and document patient progress.
• Collaborate with our interdisciplinary team to coordinate patient care and services.
• Educate patients and families on disease management, prevention, and self-care techniques.
• Monitor and report patient status to the healthcare team, including changes in condition or medication needs.
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
