Job summary

Responsibilities

Deliver patient-centered nursing care in long-term care and rehabilitation settings.
Assess, monitor, and document residents' health conditions and progress.
Administer medications and treatments per facility protocols.
Collaborate with physicians, therapists, and other professionals to implement individualized care plans.
Educate residents and families about care plans and health conditions.
Maintain accurate documentation in compliance with state and federal regulations.
Foster a safe, comfortable, and therapeutic environment for residents.
Supervise and mentor nursing assistants to ensure quality care.
