Qualifications

• High School Diploma or equivalent
• Valid driver's license and ability to drive an agency vehicle
• Ability to pass Act 33/34/73 clearances
• Pre-employment drug screening, physical exam, and TB test
• Completion of Department of Public Welfare Medication Training within 90 days of hire

Responsibilities

• Personalized Support: Develop and implement individualized care plans reflecting each resident’s needs and preferences.
• Daily Living Assistance: Help with personal hygiene, meals, medication administration and household tasks to encourage independence.
• Behavioral Support: Collaborate with the care team to apply positive behavioral strategies.
• Community Engagement: Organize and participate in recreational outings and social activities for inclusion.
• Team Collaboration: Work with healthcare professionals, therapists, and staff to coordinate care and share progress observations.
