Job summary

InterCommunity, Inc. is a Federally Qualified Health Center Look-Alike (FQHC LA) in Hartford, CT, seeking a Full-time Residential Support role.

The position focuses on milieu supervision, care coordination, and social-rehabilitation services across multiple centers in East Hartford, Hartford, and South Windsor. A CPR certification and a valid driver license are required, with 3 years of residential experience preferred.

Qualifications

• Peer Support Specialist Certification, or an Associate's or Bachelor's Degree in Human Services Field.
• 3 years of experience in residential setting preferred.
• Working knowledge of substance abuse and/or mental health treatment.
• Proof of CPR Certification within 60 days of start date.
• Valid Driver License.

Responsibilities

• Provide milieu supervision of the program and participants; coordinate and facilitate social-rehabilitation groups and programs.
• Conduct individual and group social-rehabilitation and care coordination with a caseload of identified clients.
• Supervise participants during on-site and off-site activities; perform scheduled safety checks.
• Maintain confidential progress notes and documentation on client activities.
• Provide approved transportation and run program-related errands.
• Attend mandatory staff meetings and trainings; paid as work time.
