This full-time, community-facing role focuses on connecting people with healthcare and necessary resources, addressing social determinants of health, and improving access to care. Evening availability is essential; regular evening hours are required during the annual growing season, with potential night shifts and occasional weekends.

Qualifications

High school diploma or equivalent required; Associate’s degree preferred.
Must obtain CHW certification within 6 months of hire.
Valid North Carolina driver’s license with acceptable driving record.
1–3 years of experience in a clinical or healthcare setting preferred.
Bilingual in English and Spanish preferred.

Responsibilities

Conduct outreach activities such as home visits, community education, and health screenings.
Assist with enrollment efforts for programs like Medicaid and Healthcare.gov.
Document outreach activities and enrollment in the EHR, ensuring HIPAA compliance.
Coordinate care navigation, linking patients to medical, behavioral, and social services.
Maintain patient records and complete documentation in NextGen and related systems.
Schedule appointments and communicate with patients in a linguistically appropriate manner.
