Job summary

BrightPath is seeking a Program Coordinator to lead on-site care for adolescents with intellectual/developmental disabilities and challenging behaviors in a 245D setting. You will supervise staff, manage meds, and ensure care plans are followed in a small, home-like environment.

Full-time, 4 days/week (2:00 PM–10:00 PM with every other weekend) and on-call rotation; $30/hour. Competitive benefits after initial period and opportunities for advancement.

Qualifications

• Experience supporting a vulnerable population in healthcare or oversight.
• Leadership experience or training in disability or behavioral services.
• Minimum 50 hours of education and training related to human services and disabilities.
• Ability to pass a DHS background study and maintain qualified status.
• Current valid driver's license and insurance.
• Comfort with the physical demands of direct support and transfers.
• Willingness to participate in the on-call rotation.

Responsibilities

• Provide direct care per Support Plan and ADLs, medical needs, and mental health supports.
• Be the on-shift lead; train new staff on care plans and active treatment plans.
• Own medication management and coordinate with the RN.
• Document ISP data, daily logs, and quarterly progress reports.
• Participate in IDT meetings and care planning.
• Ensure 245D/licensing compliance; manage meals and household budget.
