Job summary

Premise Health is seeking a PRN Physical Therapist for its Health Center in Princeton, IN. The role covers Monday–Thursday, 7:00 PM–11:00 PM with potential to move to part-time for regular coverage.

You will collaborate with medical staff to promote health and wellness for onsite employees and dependents, delivering high-quality physical therapy services. Required are state licenses, a degree from an accredited PT school, and CPR/BLS certification, plus at least three years of outpatient

Qualifications

State licenses/certifications to practice (where applicable).
Current board certificate (if applicable).
CPR/BLS certification with hands-on skills verification.

Responsibilities

Collaborate with Health Center staff (doctors, NPs, PAs, nurses, wellness coaches) to promote health for onsite employees and dependents.
Evaluate, assess, and develop individualized treatment plans per patients' values and current PT standards.
Promote direct access to PT services and practice at the highest skill level.
Document and report to onsite and community referral sources; participate in meetings.
Engage in continuing competence activities (peer review, presentations, grand rounds).
Interact with stakeholders (safety officers, ergonomists, HR) to promote employee safety.
Develop and deliver injury prevention and health promotion programs for onsite staff.
