Job summary

Premise Health is seeking a PRN Physical Therapist to provide high-quality outpatient care at our Health Center in Princeton, Indiana. You will collaborate with physicians, nurses, and wellness staff to evaluate and treat onsite employees and their dependents during evening shifts.

The role supports direct access to PT services and emphasizes patient-centered care, with opportunities to transition to part-time for regular coverage.

Qualifications

• Active state license to practice physical therapy.
• Current board certification if applicable.
• Degree from an accredited physical/occupational therapy program.
• CPR certification with hands-on skills verification.
• At least three years of outpatient PT experience; outpatient orthopedic experience preferred.

Responsibilities

• Collaborate with Health Center staff to promote health and wellness for onsite employees and dependents.
• Assess, evaluate, and develop individualized PT treatment plans.
• Promote direct access to PT services and work at highest skill level.
• Maintain patient-centered care and document/report to referral sources.
• Engage in continuing competence activities and potential grand rounds.
• Work with client stakeholders to support employee safety and wellness.
