Job summary

Children’s Health in Plano, TX is seeking a Clinical Social Worker (MSW) for a full-time on-site role. The position requires a Master’s degree and LMSW license, with LCSW preferred, and 2+ years of social work experience (4+ years preferred for some tracks).

You will assess families, support adjustment to illness, and coordinate discharge planning. The role involves working Mon-Fri, 2:30PM-11PM, within the Social Work department, with a focus on comprehensive psychosocial care and collaboration

Qualifications

• Masters Degree required.
• LMSW license required; LCSW license preferred.
• 2+ years social work experience preferred; 4+ years preferred for some roles.

Responsibilities

• Perform biopsychosocial screening and assessments for families to meet healthcare needs.
• Facilitate parent/child adjustment to illness and hospitalization.
• Coordinate anticipatory discharge planning to arrange home care.
• Coordinate and direct multidisciplinary patient care conferences.
