Job summary

NewYork-Presbyterian is seeking a Health Priorities Specialist – Patient Navigator for evenings to support ED patients, coordinate their journey from the ED to discharge, and connect them with the care and services they need.

Requirements include a Bachelor's degree, fluency in Spanish and English, and experience in primary or community health; knowledge of hospital and community resources; and strong culturally sensitive customer service skills.

Qualifications

• Bachelor's degree required.
• Fluency in Spanish and English.
• Minimum of years’ experience in primary or community health.
• Knowledge of overall hospital and community resources.
• Diverse and culturally sensitive customer service skills.

Responsibilities

• Work as part of the ED health care team as a patient ally throughout the continuum of care.
• Coordinate the patient experience, from ED to after discharge.
• Connect patients to care, including the specialist or service they need.
• Support patients to navigate the health care system, including telehealth, and educate on health issues.
