Job summary

Qualifications

• Direct patient care including ADLs, feeding, hydration and elimination support.
• Safely assist with transfers, positioning, ambulation and fall prevention.
• Monitor and document vital signs and report changes to RN/LPN; document in EHR.
• Maintain clean, safe environment and adhere to PPE policies.

Responsibilities

• Direct Patient Care (ADLs): Assist patients with personal hygiene, nutrition and elimination needs.
• Mobility and Safety: Safely assist with transfers, positioning and ambulation using proper mechanics and assistive devices.
• Observation and Communication: Accurately monitor vital signs; report changes promptly; document in EHR.
• Environmental Care: Maintain a clean, safe, orderly environment; ensure medical equipment functions; follow infection control policies.
