Job summary

Northwestern Medicine is seeking a licensed clinical social worker (LCSW) to join our hospital social work team. The role focuses on psychosocial assessment, discharge planning, and resource coordination across patient populations.

You will collaborate with care teams, provide crisis intervention, and advocate for patients and families throughout the care continuum. Strong communication and case management are essential.

Qualifications

• Licensure in Illinois; Licensed Clinical Social Worker (LCSW).
• Master's Degree in Social Work from CSWE-accredited program.
• Minimum of two years post-graduate hospital social work experience.
• Strong interpersonal skills to achieve positive outcomes.
• Organizational skills to manage caseload and daily work.
• Analytical skills to collect, analyze and interpret data and navigate sensitive situations.

Responsibilities

• Meets directly with patient/family to assess social, emotional, cultural, mental status, environmental and financial circumstances.
• Provides psychosocial interventions including crisis intervention, discharge planning and community linkage.
• Collaborates with interdisciplinary teams to coordinate plan of care and ensure timely discharge.
• Participates in case management activities, performance improvement, and documentation in medical records.
