Job summary

Clermont Park Community is seeking a compassionate Part-Time LPN for evening shifts (4pm-10:30pm). The role runs Week 1: Mon, Thu, Fri; Week 2: Sun, Mon, Thu, Fri. Pay ranges from $33 to $39 per hour, with a strong emphasis on patient-centered care and team collaboration.

The LPN will provide direct nursing care to residents, administer medications, monitor conditions, and document changes for the care team. Evening hours and rotating weekends require reliability and strong communication.

Qualifications

• 1-year experience in a long-term care facility preferred.
• 1-year experience in overseeing and monitoring other staff preferred.
• IV Certification preferred.
• Must have computer word processing and database experience.
• Must be able to read, write and speak the English language.
• Must possess excellent customer service skills.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor and document residents' conditions, reporting changes to the care team.
• Support residents with daily care needs in a respectful, person-centered way.
• Collaborate with nursing staff, aides, and the interdisciplinary team.
• Respond to resident needs and emergencies during the shift.
• Maintain accurate and timely documentation.
