Job summary

Clermont Park Community in Denver, CO is seeking a part-time LPN for evening shifts (4:00 PM – 10:30 PM), with rotating weekends. The role involves providing direct nursing care to residents and collaborating with the care team to support health, comfort and dignity during the evening hours.

You will administer medications, monitor residents, document accurately, and work both independently and as part of the team.

Qualifications

• Active LPN license in good standing
• 1-year experience in a long-term care facility preferred
• 1-year experience in overseeing and monitoring other staff preferred
• IV Certification preferred
• Must have computer word processing and database experience
• Must be able to read, write and speak the English language
• Must possess excellent customer service skills

Responsibilities

• Administer medications and treatments as prescribed
• Monitor and document residents' conditions, reporting changes to the care team
• Support residents with daily care needs in a respectful, person-centered way
• Collaborate with nursing staff, aides, and the interdisciplinary team
• Respond to resident needs and emergencies during the shift
• Maintain accurate and timely documentation

Education

Colorado State Licensed Practical Nurse license, in good standing

1-year experience in a long-term care facility
