Job summary

Qualifications

• Master’s degree in social work from CSWE-accredited program.
• LMSW/LCSW licensure in Delaware required.
• Minimum 6 months social work experience.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families.
• Utilize advanced clinical judgment to identify risks and interventions.
• Assess safety, abuse, neglect, self-harm, domestic violence, substance use, and social determinants.
• Independently manage high-volume caseloads with emergent needs.
• Provide crisis intervention, counseling, and resource coordination.
• Collaborate with interdisciplinary team to develop patient-centered care plans.
• Facilitate referrals to community resources and behavioral health services.
• Maintain accurate documentation in the EMR.
• Demonstrate sound decision-making in changing circumstances.
