Job summary

Jefferson Health is seeking an experienced Registered Nurse, Coordinator of Patient Care Services, to lead daily unit operations and act as a clinical role model for the patient care team.

You will evaluate patient needs, coordinate staffing, and collaborate with medical staff to ensure compassionate, safe, and high-quality care across shifts. Strong emphasis on patient-family partnerships and continuous professional development.

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• 3 years as a Registered Nurse in an acute care setting.
• Effective Communication—understanding of effective communication concepts, tools and techniques.
• Patient-and Family-Centered Practice—knowledge of how to convey respect and collaborate with each patient.
• Cultural Awareness—knowledge of diverse backgrounds and ability to connect.
• Care Coordination—knowledge of medical, social, economic, and other services to integrate treatments.
• Patient Safety—knowledge of risks and steps to reduce them.
• Medication Administration and Prescription—knowledge of meds and administration.
• Discharge Planning—understanding post-discharge needs and planning follow-up.
• Assessment and Diagnosis—tools for patient assessment and nursing diagnosis.

Responsibilities

• Interact with co-workers, visitors and staff per Jefferson values.
• Communicate with interprofessional teams and services; serve as a clinical resource.
• Collaborate with medical staff to ensure consistency of care plans.
• Assess patient needs and make care recommendations based on physical, psychosocial, and cultural needs.
• Coordinate admissions, transfers and discharge optimization for the unit.
• Provide scheduling data and manage overtime or shifts as needed.
• Evaluate staff performance and support changes in clinical practice.
• Participate in nursing development programs and mentorship.
